Job SummaryUT Health Tyler is seeking an experienced Acute Care Registered Nurse Case Manager to join our team. As a key member of our healthcare team, you will be responsible for coordinating and advocating for our patients' care, creating an appropriate discharge plan, and reviewing patients' charts to establish the most suitable course of action.Responsibilities

Coordinate and advocate for our patients' care and create an appropriate discharge plan.
Review patients' charts and meet with families and healthcare providers to establish the most suitable course of action.
Remain in contact with patients and their families during the hospitalization stay.Qualifications
- Current RN license in the state of Texas.
- Registered Nurse with an Associates, Bachelor's or Master's Degree.
- Minimum of three (3) years of clinical experience.Preferred Qualifications
  - Case Management certification.
  - Experience in utilization review, discharge planning, quality programs.
